We have a vacancy for an Advanced Level Pharmacist - Crisis Pathway and Home Treatment Team. We are looking for someone passionate about mental health, a highly motivated and enthusiastic individual with clinical experience in mental health medicines optimisation to join our expanding pharmacy team.

The role will involve provision and development of patient centred clinical pharmacy services to the Crisis Service and Home Treatment Team as part of the multidisciplinary team. The successful applicant will also act as a role model for Junior Pharmacists undertaking their Clinical Diploma, providing support and supervision within the team.

To be a non-medical prescriber is essential, as there is a requirement to prescribe for patients, as part of the multidisciplinary team.

Clinical supervision will be provided as part of this role in developing further expertise in Adult Mental Health.

Main duties of the job

The aim of this role is to support development of the teams involved in the Crisis Pathway including Home Treatment Teams (HTTs), Acute Crisis and Assessment Teams, and the integrated Care Hub within NELFT, to help facilitate the management and treatment of patients experiencing an acute mental health illness, to either reduce and prevent admission, or facilitate safe discharge, ensuring patients transition safely back into primary care. The post holder will need to be able to provide expert pharmacy and medicines management advice to optimise patient’s medication regimes, advise on appropriate management strategies and safely identify and manage risks.
